To be considered for the State of Ohio Cannabis Card, the patient must be diagnosed with the following qualifying medical conditions:

 

  • AIDS, positive status for HIV,

  • Amyotrophic lateral sclerosis

  • Alzheimer’s disease

  • Cancer

  • Chronic traumatic encephalopathy

  • Crohn’s disease

  • Epilepsy or another seizure disorder

  • Fibromyalgia

  • Glaucoma, hepatitis C

  • Inflammatory bowel disease

  • Multiple sclerosis

  • Pain that is either chronic and severe or intractable

  • Parkinson’s disease

  • Post-traumatic stress disorder

  • Sickle cell anemia

  • Spinal cord disease or injury,

  • Tourette’s syndrome

  • Traumatic brain ​injury

For your appointment, medical records with  confirming diagnosis must accompany you visit for Dr, Hom's evaluation.  Your office visit is not covered by insurance with estimated cost to be approximately $150.00 to $175.00.
